Umpire Incident Report

A report must be submitted to the Supervisor of Umpires and the League President after any game in which violations of rules occur and other incidents worthy of comment, including disqualification of any trainer, manager, coach or player.

Send the original report to the Supervisor. A second copy goes to the League. A third copy goes to the Umpire.
